What It Is:
We make a commitment to all the groups and their participants to ensure that all healthcare dollars are used efficiently and effectively. To this end, we have designed a user-friendly, consumer friendly program called pre-notification. Pre-notification is an early opportunity to initiate disease management and screen for large case management.

How It Works for You:
When a healthcare provider contacts the claim department for pre-notification, our claims staff gathers data about the patient's condition. Based on the condition, the case management nurses may contact the patient and/or physician to determine if the case can be managed. Participation in the case management program is solely the patient's decision.

With the patient data in hand, a comprehensive team of special care nurses creates a plan of continued care services to address the patient's overall condition. Using this strategy, the team is able to save health plan dollars while improving the quality of the patient's care. This feature is most commonly utilized in cases involving diabetes, asthma, and cardiac care.

The ISTA FSP Insurance Trust formed a partnership with this independently owned, managed care service to help defray your plan costs. Your group's savings – the hard cost savings – are reported every quarter. The savings for last year totaled $8 for every $1 spent.
