ISTA Insurance Trust
The ISTA Insurance Trust is a tax-qualified voluntary employees’ beneficiary association trust that provided the funding for medical, long-term disability, dental, vision, and life benefits for employees of school corporations that participated in the Trust. For the most part, Trust operations have been suspended. Following is a status report on each of the benefit programs that were funded through the Trust.
Medical
- Program ended effective as of June 30, 2009; no medical coverage through the ISTA Insurance Trust after that date.
- For information pertaining to claims incurred before July 1, 2009, or for plans that did not renew with UnitedHealthcare as of July 1, 2009, call (866) 996-2048.
- For information pertaining to claims incurred after July 1, 2009, call (800) 357-0978.
Long-Term Disability
- Program ended effective as of July 31, 2009; no LTD coverage through the ISTA Insurance Trust after that date.
- The administration of the LTD program has been transferred to Prudential Disability Services.
- All questions regarding payments, existing claim status, potential claims, etc. should be addressed to:

Dental
- Program ended effective as of July 31, 2009; no funding of dental benefits through the ISTA Insurance Trust after that date.

Vision
- Program ended effective as of July 31, 2009; no funding of vision benefits through the ISTA Insurance Trust after that date.

Group-Term Life and Paid-Up Term Life
- This program continues to be funded and administered through the ISTA Insurance Trust.
